We are recruiting a Extra Care Team Leader on a part-time basis including evenings, weekends and Bank Holidays.
Within this role you will be required to:
· Complete rostering electronically and ensure effective building cover.
· Chair meetings with staff members
· Complete person-centred care plans
· Take on duties of home carer when required
· Report and record accurately.
· Support team manager in recruitment and retention.
