Job Advert

We are seeking a motivated and Reablement Team Leader to join our team. This role is focused on supporting individuals to regain independence and achieve the best possible outcomes in their own homes.


Key Responsibilities:

  • Plan and manage staff rotas to ensure effective service delivery.
  • Assess customers needs and complete comprehensive risk assessments.
  • Identify and arrange appropriate equipment and support to help customers regain and maintain independence.
  • Monitor customer records and documentation, ensuring accuracy and compliance.
  • Review care packages regularly and identify when amendments or additional support may be required.
  • Conduct staff supervision meetings and
  • Personal Development Reviews.
  • Carry out observations of staff practice, providing coaching, guidance, and constructive feedback.
  • Promote high standards of care, safeguarding, and person centred support at all times.
  • Work collaboratively with customers, families, healthcare professionals, and other stakeholders to achieve positive outcomes.
  • Experience working within reablement, home care, or adult social care services.
  • Strong leadership, organisational, and communication skills.
  • Knowledge of risk assessment processes and person centred care planning.
