Are you a passionate and dedicated Social Worker looking to make a real difference in the lives of older adults? Join Bedford Borough Councils Adult Social Care team and be part of a supportive forward thinking environment committed to promoting independence and choice as well as wellbeing.

 

We are seeking a qualified and experienced Social Worker to join our Older Peoples Team. You will work with adults who have complex social care needs whilst carrying a varied caseload with increasing complexity and autonomy. You will contribute to safeguarding, assessments, care planning, and service delivery in collaboration with health and care partners. You will play a vital role in promoting personalisation, ensuring services are shaped around the individual, supporting independence and enabling older people to live fulfilling lives.

 

Key Responsibilities: Undertake assessments under the Care Act 2014 and other relevant legislation. Coordinate and review person centred care plans. Lead safeguarding enquiries. Liaise with service users, carers, families and multiagency professionals. Provide crisis intervention and emergency response where necessary. Maintain accurate records and contribute to service evaluation and planning.

 

This post carries a market rate supplement of £2,000pa pro rata (subject to review), a welcome payment of £6,000, and retention payments of £2,000 paid after 4 years service and then every 4 years subsequently.
