We are seeking a motivated and Reablement Team Leader to join our team. This role is focused on supporting individuals to regain independence and achieve the best possible outcomes in their own homes.

Key Responsibilities:
Plan and manage staff rotas to ensure effective service delivery.
Assess customers needs and complete comprehensive risk assessments.
Identify and arrange appropriate equipment and support to help customers regain and maintain independence.
Monitor customer records and documentation, ensuring accuracy and compliance.
Review care packages regularly and identify when amendments or additional support may be required.
Conduct staff supervision meetings and
Personal Development Reviews.
Carry out observations of staff practice, providing coaching, guidance, and constructive feedback.
Promote high standards of care, safeguarding, and person centred support at all times.
Work collaboratively with customers, families, healthcare professionals, and other stakeholders to achieve positive outcomes.
Experience working within reablement, home care, or adult social care services.
Strong leadership, organisational, and communication skills.
Knowledge of risk assessment processes and person centred care planning.

We are currently seeking a Reablement Support Worker to join our successful, motivated and committed team.

Within this role, you will make a great step in a long lasting career and make a difference to people's lives.

This role will allow you to work in a caring and supportive role with adults who live in their own home, help them to become independent with all aspects of daily living, in a supportive, experienced and motivated team.


The successful candidate will be provided with mandatory training, support and guidance from our committed, motivated, energetic and professional team coordinators. You will receive supervision, appraisal and comprehensive induction.

In this role you will need to travel between service users homes within a given timescale. You will need a current UK driving licence and access to a car.


The role provides hands on personal care to service users . You must be able to carry out physical care tasks. Manual Handling training experience preferred but training will be given.
Reablement promotes a culture of independence and has a strong teamwork ethic.

The service runs between 07.30 and 22.00, 365 days a year.

We are currently looking to recruit a part time Visual Impairment Rehabilitation worker for our Sensory Impairment Team.

Reporting to the Advanced Practitioner, the successful post holder will be responsible for providing a professional rehabilitation service to adults and children with a visual impairment, including advice, mobility training, daily living skills and communication skills.

You will be required to hold a diploma or equivalent in Rehabilitation studies. We would welcome candidates who also have rehabilitation qualification to apply.
You will have extensive experience of working with adults and children with visual impairment within the community.
You will be able to work autonomously and as part of team as well as multiagency working to ensure that the health and wellbeing of adults and children are paramount to service delivery and help to deliver excellent services to the residents of Bedford and Central Bedfordshire Boroughs.
You will have excellent verbal and written communication skills, as well as excellent computer literacy to maintain good case recording.

We offer 27 days of annual leave, increasing to 32 days after 5 years service, plus bank holidays, compensated by a competitive salary and a generous pension scheme.

We are creating a new Family Group Decision Making (FGDM) Lead role to design and establish this approach across Children’s Services. You will lead from the ground up, shaping how FGDM is implemented, embedded and measured across the system.

You must bring strong experience of Family Group Conferencing or similar family-led decision-making models. This is essential. You also need a proven track record of setting up or leading new services, programmes or system change in a multi-agency environment.

You will define the model, build practice standards, support practitioners, and ensure high-quality delivery that strengthens family networks and improves outcomes for children. You will be working to develop this service as part of our work towards the national Family's First reforms. You will need to be able to deliver FGDM yourself and lead others in doing so. This is great opportunity to build something from the beginning.
